 Your Life Care Planning Checklist-
This document lists out all the forms in the packet so that you can check off which ones you have completed. If you wish to register your documents with the Arizona Health Care Directives Registry, the checklist will let you know which forms are accepted. Upon completion of your Online Notarization, you will be asked if you would like for your Notary to file the forms for you via email. 

 Registration Agreement-
This form allows you to register the documents with the Arizona Department of Health
 
 Health Care Power of Attorney-
This form allows you to select a person to make future medical decisions for you if you become too ill to communicate or cannot make those decisions for yourself.

Living Will- 
This form allows you to list out the type of medical treatments you do or do not want for your end-of-life care. It should go with your Health Care Power of Attorney form, so your agent knows your wishes.
 
Mental Health Care Power of Attorney-
This form allows you to select a person to make future mental health care decisions for you in case you become incapable of making those decisions for yourself.
 
 Prehospital Medical Care Directives (Do Not Resuscitate)-
This form needs to be on orange paper and should be signed by you and your doctor. It informs emergency medical technicians (EMTs) or first responders not to resuscitate you. Sometimes this is called a DNR – Do Not Resuscitate. Please note this is valid prior to going to a hospital, if admitted to a hospital they may require you to fill out another form for their hospital.  Registration Agreement o If you would like to register your documents with the Arizona Health Care Directives Registry, you MUST fill out this form and submit it with your documents. PLEASE NOTE THAT THIS DOCUMENT MUST BE PRINTED ON BRIGHT ORANGE PAPER AS IT APPEARS ON SCREEN. YOU MAY WATCH THE VIDEO ABOUT THIS DOCUMENT HERE:

What You Will Need Prior To Scheduling (Please Read Carefully):

  • You will need to download and read the Advanced Care Planning Forms from the State of Arizona here: CLICK HERE TO DOWNLOAD

  • On the day of your appointment, you will be able to fill out the documents online and have them Notarized. PLEASE DO NOT FILL OUT THE FORMS PRIOR TO THE NOTARY SESSION. YOUR NOTARY WILL HAVE THE FORMS UPLOADED FOR YOU. YOU SHOULD ONLY ALREADY KNOW YOUR ANSWERS PRIOR TO THE SESSION. 

  • You will need access to a desktop, tablet, or smartphone that has good quality camera and audio (absolutely no exceptions).

  • You will need your non-expired US driver's license or United States Passport in order to have a document notarized online.

    • However, if you are in need, you may contact us for a referral to an out of State Notary in Virginia who can Notarize your documents for you (which subject to price changes).At this time, we are not able to offer Remote Online Notarizations to those who have foreign drivers' licenses or 

  • You will be required by the software used for Online Notarization to answer a series of questions, similar to a soft credit pull that will not affect your credit to help verify your identity. This is called KBA, or Knowledge based assessment. 

  • You must be of sound mind and body and willing to sign the documents at the time of the Notarization. You will not be able to debate the documents at the time of the Notarization in any way or the Notary will have to cancel the session. For this we strongly advice downloading the documents prior to the session and making sure you know your wishes prior to the session. 

  • We do allow you to fill in the information online but are not able to guide you through the process. A Notary's job is to only witness the signatures of the signers after verifying their identify. Notaries are not attorneys and therefore cannot provide legal advice.
